Fighting Shadows is dangerous work, so it helps to have a doctor who can provide the health items and consumables it takes to survive. Fortunately, there’s a back-alley clinic a block away from Leblanc, so Joker can get all the medicine he needs from Tae Takemi, the Death confidant ofPersona 5 Royal.
Ranking up Takemi’s relationship unlocks bigger and better health items, making her one of the most essential confidants inPersona 5 Royal. She’s also one of the first confidants Joker can build up, so players should get started on this relationship as soon as they can.

General Information
The infamousDeath Arcana of the Tarot deckdepicts a battlefield, a decapitated head looking to the left, and other symbols of death like a scythe, a sword, and a skeleton. The card can refer to a sudden transition, a great loss, or an illness, while reverse meanings include stagnation or a partial transition.
As an essential merchant, players can find Takemi in her Yongen-Jaya clinic every day, and her rank events can also happen onany daywhen Joker has time to spare. The only restriction is that players must wait at least three days since the last rank event.
Takemi doesn’t like to go out anywhere, but Joker can spend time with her if players don’t have enough relationship points to see the next rank event. During these hangouts, Joker cangive Takemi a gift for extra points, and her favorite gifts are theHeart Ring,Mini Cactus,Black Mug, and48-Sided 3D Puzzle.
Takemiis also a potential love interest. If players choose the romance route and reach rank 10, they can invite Takemi to spend Christmas Eve together. Her present is aMuffler, and in New Game Plus runs this item will give players extra points from the same-Arcana bonus. Joker can also spend time with Takemi on Valentine’s Day to get Takemi’s Chocolate (an item that restores all SP to one ally), and date her on White Day.

Players can start the Death confidant as early as 4/18, which is when Morgana suggests asking the doctor about her special medicine. Most of the dialog choices don’t matter, but the last one can give players a few starting relationship points.
Players can get an extra point by holding a Death Persona in their stock, and the point values in this guide will assume that players are doing so. For establishing the Death confidant, Joker unlocksRejuvenation, which adds Takemedic-All and Takemedic-All V to Takemi’s inventory. These items restore 50 or 100 HP to all allies.

Before starting this event,Joker will needGuts 2 (Bold). During the second clinical trial, an officer shows up to harass Takemi.
For getting through the trial, players are rewarded with asmall Guts boost.

The next trial gets interrupted by a phone call, and Takemi mentions her bad reputation in the larger medical field.
Joker gets anothersmall Guts boostand unlocksSterilization. This adds Rejuvenating IV, Kajaclear-R, and Kundaclear-R to Takemi’s inventory. The first item restores HP in safe rooms while the other two itemsclear enemy buffs and ally debuffsduring combat.

A young girl suffering from a rare disease shows up in the clinic one day, and Joker volunteers Takemi to take care of her.
The rank reward is anothersmall Guts boost.
Rank 5
Takemi starts opening up about her past, and she explains how she lost her last job at a hospital.
Joker gets anothersmall Guts boostandImmunization. This is a big one, because it unlocks a set of accessories offered exclusively at Takemi’s clinic: Regen Patch 1, 2, and 3, and SP Adhesive 1, 2, and 3. These items are expensive, but equipping them gives the characterthe passive skills Regenerate or Invigorate. Takemi also adds Takemedic-All Z, which restores 200 HP for all allies, and Life Ointment, a full-HP revive item.

This event takes place outside Takemi’s clinic, which means it can’t happen on rainy days. Takemi is starting to get locally famous since the father of the girl from last time went out singing her praises.
Joker gets anothersmall Guts boostfrom this rank.

Word of Takemi’s practice gets back to Oyamada, the man who ruined her career. He’s back to threaten her to get out of the medical business for good.
Joker gets anothersmall Guts boostfrom the clinical trial, and for sticking up for her Takemi gives Joker aDiscount. All items at the clinic, including the accessories, are now 50 percent off.

The next event doesn’t improve Takemi’s rank, but players need to see it to continue this confidant. First, however,Joker will needCharm 4 (Charismatic)to get through to Takemi.
This event will unlockthe Mementos request “Bad Medicine,“and players must clear it to continue with the Death confidant. Oyamada’s Shadow is on Chemdah Area 2, and it turns into Thoth. Thoth has a strong or better affinity with every magic element, so focus on Physical attacks to take him down. After the fight, Oyamada’s Shadow drops aMediarama Skill Card.

During the next rank event, Takemi will get some incredibly good news now that Oyamada has had a change of heart.
This rank comes with anothersmall Guts boost.
Rank 9
The tailored medicine is finally ready, so Takemi sends it off. This eventtakes place at Inokashira Park, so it’s unavailable on rainy days.
If players are on the romance path:

The rank 10 event is completely different on the romance and friendship paths. For one thing, the romance event takes place at night, while the friendship event takes place during the day as usual. Relationship points no longer matter, but players can get the best results from choosingany, 2,and3.
The final confidant ability isResuscitation. Takemi’s inventory expands to include Reviv-All (revive all allies to half health), Renew-All (revive all allies to full health), and both Magic and Physical Ointments (these items create magic or physical barriers that reflect attacks). Finally, Joker gets access toAlice, the most powerful Death Persona.
On 3/19, Joker can stop by the clinic to meet with a max-rank Takemi and get herDog Tag.During a New Game Plus run, players with this item will get full access to Takemi’s inventory once they restart the Death confidant. However, this doesn’t include the discount.
